Simple to use and very functional for my unique medical practice with unique needs.

The form is difficult to work with. If you delete something a gap is created in the form and there appears to be no way to fill the gap. Moving a section within the form doesnt work.

I was looking for an app for contracts, with placeholders or dynamic fields. Every other app on the app store for this has been abandoned or not updated and is non working. This is great to make a pre populated form, easy to tweak if needed, can save without boxes in the final form so it looks more like a document and not a spreadsheet! No annual subscription, free initial app, but I bought the pro version for field calculations. APP developer updates frequently to keep up with IOS changes. ONLY thing missing for me is a native contact import field, until then Im copying and pasting all info into a text box. Also would like a theme with a plain white background, no clip or notebook shown when a client signs a form. Change background where folders are kept would be cool, but minor. Ive tried all the form maker apps, take my word for it this is the most customizable updated app in the app store, thank you!

Ive been using the Form Connect app for several years now. It is an excellent app for data collection. Far easier to use than other form apps that I have tried. Any issue that has come up in the past has been worked through with Steve...sometimes realizing the issue was on my end and sometimes resulting in new features being added to the app. I look forward to seeing what features lie ahead.

Ive used this app for years on iPads but now that Ive upgraded to the iPad Pro the FormConnect app seems to have been broken. I can still get to the forms and use it but the screen layout is severely messed up. Hopefully this will get fixed soon.

New Forms & Edit Options Disappear whenever you delete a form. Closing and reopening program does not restore the options. Rebooting the phone will restore the options. Please correct this glitch and Ill increase my rating.

Started using this app and now all my inspectors use it to send out inspection reports with pictures. Easy to use and has all kinds of abilities just keep looking around and you find them. My inspectors are tech challenged and they love the forms I created for them with this app.
