Ive been using my custom forms from FormConnect for almost a year. It WAS awesome. Now, after a few iOS updates, it is has become unstable. Day after day, I check my updates, praying there will be one for this app. Sadly, the developer has ignored the paying customer. Issues have been mounting through the last 2 major system updates, with no hope in sight. Another great app which is now dead along the highway. Struck down by progress and neglect.

I’ve been using this form program in my emergency dept for several months and I love it. I can modify the form at any time as I find new items to add to my patient tracking program. Using it on iPad Air and iPad mini. Wishing it had some decision tree support as well as checkbox to ‘prose’ options which I have suggested to the company.

I was excited to use this app for my business. I have clients that have to fill out waivers & release forms for services & was tired of printing out the forms. I thought it would be great to be able to have an electronic version that I could send my clients prior to their appointment so they could fill them out & sign them & return back to me. Saving time & paper. The description says "be able to share with others". What it doesnt say is that the "others" must have an iPad & FormConnect on their iPad to be able to complete it. Very disappointed!! Also the customer service response when I asked a few questions was very poor. Basically a one sentence reply with best regards, then the reps name. If you need this app for anything like I did, look elsewhere do not waste your time or money. All my normal forms are in word format & there is not a away to import these to formconnect. You HAVE to start from scratch creating fields, labels, text the entire form has to be designed.

Nice layout once a form is made...but needs a lot of work. Bought the pro version prematurely. 1. Needs to be a form manager (ordered and filed) so your opening screen isnt just a bunch of random forms. 2. Are forms limited to one page? Should be able to scroll down and make them as long as needed. 3. Should be able to select multiple answers for radio buttons - otherwise why have them AND drop downs. 4. Should be able to auto populate different forms with specific information from the last form for the same subject. Bought this with high hopes of documenting subjects for studies. Not a real database app - yet

As an environmental scientist, I use this app for collecting data in the field. I basically created versions of my own standard forms in the app and then I can export the data via a csv. File can manipulate the data in excel. What I would like to see is the ability to change back to "no selection" in a drop down tab in case I accidentally hit the wrong button and the ability to tag data to a GPS location. Also, if they added the ability to create/edit forms outside of the app on a PC/Mac , this would be a 5 star. It takes a little bit of time to set up your form the way you want it, but after you do it is incredibly easy to manipulate. Definitely worth a try (Ive been using it for almost 2 years now).

Crashes all the time!

I use this on our iPads for our technicians to fill out inspection forms created while at our customers site and it can be electronically signed, typed no messy handwriting anymore and emailed to customer and PC for permanent files. Our customers love it over our old carbon copy forms. Its a great business tool

We use FormConnect for building in-home bids for our environmentally friendly housecleaning service, and being able to go paperless is incredible! Not only does the app support our drive to be green, it just flat out works. It is very customizable, but more importantly, it is really intuitive. Great app!

I like the ability to create forms based on my needs. I upgraded a year ago to formconnect pro. I keep having problems with the app crashing in the middle of use with clients - this is extremely inconvenient and embarassing. PLEASE FIX THIS GLITCH. Customer service could be friendlier - I wish there was an option for providing suggestions. Ive emailed suggestions and have received defensive emails in response. I would recommend allowing suggestions or surveying users for needs. Id like to see the ability to select all and change font size or characteristics for text that has already been entered. Id also like to be able to either "hide" templates that I dont currently need or arrange templates in a preferred way instead of it automatically going alphabetical. Finally, it would be great if a specific text field could be set to carry over into multiple pages (e.g. customer name) and if pages numbers could be added (e.g. page x of y).

Im so glad I found this app! I used to use Bento to keep track of sooo many things and was devastated when it was discontinued. I tried a wide variety of note, database, & inventory apps, but nothing really worked for me. Then I found FormConnect. I love the book and/or clipboard style and the ability to create books/files to keep track of various things regarding which I need different information. I would give it 5 stars if it had the following features: 1. I really HATE the background on the initial screen where the files/books are displayed and would LOVE to be able to substitute a solid background. 2. I would love to be able to use a wider variety of book and paper colors & to choose which paper goes w/ which book cover or clipboard. 3. I want to be able to rearrange the books/files on the initial screen. 4. Within a book or file, I want to be able to swipe from right to left or from left to right to move from one record to the next without using the list on the left, but want to keep the list on the left to navigate quickly to a given record. 5. I wish there were a basic help system to answer questions rather than having to figure things for myself. 6. When you first choose the app, you get a screen which may provide good info on using the app, but you get a succession of pop up boxes that ask whether or not to deal w/ a variety of files about which you have no knowledge. This needs to be fixed.

This is a decent form builder. Much easier to build forms with this app than in a spreadsheet like Numbers. But this app is so ugly it drives me crazy looking at it. I prefer to juggle the slow temperamental Numbers than look at this old dated UI. I really hope these guys change the interface. The way it is now makes me feel like Im using a toy instead of running a legitimate therapy business.

Versatile. Easy to use. Great customer support. Using this app on several projects at a variety of locations with multiple users. Excellent product !!

With iOS 8 the ability to export PDF file to email or dropbox is no longer available.

Since the last update, you are not able to export, was the purpose of the app.

With the ios8 update, there are features not working. Most importantly, the ability to export and send forms in PDFs via email. Please help!!!!

It works on iOS 8.0.2 in landscape mode for export...

This ap is the best ap I have ever used! I run a trucking company and can instantly create any form I need, be it bill of lading or invoice. I recently had an issue with the ap and it was immediately addressed by customer service.. They rock!

Great for what I wanted to do...make some simple forms to collect data for building maintenance. The editor is easy to use and gives a nice clean appearance to the form.

There are so many things I love about this app. But one of the most important reasons I purchased this app was so that I could have forms where I could collect and maintain client signatures long term without a paper backup. A couple of weeks after I purchased the app I also purchased a new iPad. When you transfer data from one iPad to another, everything transfers except the signatures. I asked the company for support and at first they responded, but when even the solution they offered didnt work and I asked for another option they stopped responding. Its really unfortunate because this is a pretty good app in all other ways. But not having signatures on consent forms exposes my business. This app desperately needs to be updated so that when users change iPads or have multiple devices they are using to manage forms all data including signatures transfers

Great app! some bugs with iPad keyboard not showing and a terrible map field having to use geo references instead of addresses.
