Your business needs form connect! I write a lot of estimates and keeping track of all the paperwork is no longer an issue. Emailing them from in front of there house gets a quicker response than leaving them behind and hope they get them. Its far more neat and easy to use. Buy it you will not be disapointed!

I purchased this app to make custom order forms for my home based bakery. I love being able to make customized forms and fill them out, however when I organized them by date, the date shows up year, month, then day, which to me is very confusing to get used to this format! There is no way to change it and when I contacted the makers about it they said there was no way to provide an option to change the format, which I have a very hard time believing. Over all the app will work for what I need it to do but isnt what I hoping for as far as usability after the form is filled out.

This app is good enough for MickeyMouse stuff, nothing complex.

If you work in the field and need forms this is the app to use. It is intuitive and therefore the learning curve, if any, is minimal. Support has been wonderful.

I love this App. One of the first I bought for my iPad! Great customizable forms. I have use it for many things from Game Character Sheets to Equine and Canine Record Sheets. From Custom Work orders (with sketches and signatures) to tap to enter time cards. One thing that would be a great improvement would be a swipe from record to record rather than having to tap the tiny forward/back arrows all the time.

This is a decent database and has good upgrades. The programmers seem very responsive to input. Highly recommended. Would give 5 stars but the record lock button does not seem to apply to drop-down boxes. The record lock should lock the entire record.

The most user friendly form creator I have come across. Perfect for the non statistician clinician. Does all that I need it to do. A few minor snags here and there in terms of field label sites being limited to the left of the field on a form but that is a minor cosmetic issue that isnt a limiting factor at all. A clear 5 star in my opinion.

I really like all the features since I downloaded Pro. My only request at this time is within the calculations if it could calculate in percentage as well.

I love this program. Was able to create a complicated 4 page survey with relative ease. Looks great and has a very intuitive design interface. Downgraded because in app purchase is not working.

Love the App! I can nearly run my building from my Ipad because of it. However, I have request. One, can you incorporate a audio recording feature so that I can type and save the recording of meetings held? Second, can you add a feature where we can create folders to drop specific form templates into. For instance, I have a form template called evaluations. I need a folder on the home screen of the app that says administrative files to drag and drop the form in to. Again, fine job on what I consider one of the most essential user friendly apps developed to date!

This app is not good in landscape view. If you use landscape with a keyboard/cover or just happen to like landscape prepare to loose a lot of potential screen. Developer refuses to rework to accommodate a better landscape view even when their competitor Bento does it with ease. The fields are already over padded so once the big landscape view, cut-off squeeze happens it gets hard to work with.

Ive created a 20-page Client Information form for my client. I used nearly all the types of boxes/lines/drop downs. Everything works well and my client is very happy. And when I did have an issue, tech support was fast and efficient. He even sent me an email two weeks later to let me know the update was available. Awesome customer service!

Very impressed with the ease of use and options to create work checklists, training documents and other forms.

Would give 5 stars if it had the ability to be opened in Word. Clients love signing the contract on the iPad, but cant open questionnaires without converting from PDF and not everyone can do that. Would like to be able to have different fonts though so we can better reflect our image. Also wouldnt mind having a scale for our post event questionnaires. A few more backgrounds or the ability to create your own might be a good idea. Id like to have each of our questionnaires and surveys with a different "cover" that includes our logo.

This is the first time I have written a review for an app, but this one deserves all the praise it is getting! This is an incredible app. The customizability of it in amazing. Want a drop down box here...make one...a text field there...add it. I was amazed at how easy it was to use and how quickly you could create customized forms. I am in the financial services and this application has saved me hours. I also asked a question and received a response within the hour. Amazing customer support! Highly highly recommend this app.

Ive started using this app for several different businesses Im involved with. Its easy to use and build forms. Allows signatures, easy to configure for my needs. Wonderful helpful friendly support from a live person who speaks English! Love it.

I use this app for most of my data collection functions. I purchased the Pro version. It is great for collecting and documenting issues and problems. I can import/export data for other (non-iPad) users.

This is easily the best form/database out there in terms of layout and creating a real form that guides users through the process of adding data ... And that is soooo important. Its really easy to use ... Although there is room for improvement of course, but no way Im taking stars off for that. As is, totally useable, flexible, and for one issue I had, the developer was lighting fast with a response and fix. I was very impressed. I bought the Pro update ... The Kisok mode is great if you dont want users to have access to data records... Or need calculations in the form. Global or selected records, export to csv, PDF , and form backup via DropBox and email meets my needs entirely. Sure, Id like email fields which, when populated and tapped, open a ready to go email, export of a selection of fields within the form ( rather than the whole form ), a Next key on the keyboard to jump to the next text field, maybe an export directly to Numbers on the iPad ( or iCloud)... but these are just conveniences which dont impact on the thrust of the app’s purpose. Ill email the dev with more ( probably unreasonable ) requests! If you want a really easy, transparent data collector and manager try this first!

Wonderful app that is by far the easiest to use from other form builders. The only downside is that if you have two or more devises, the app does not automatically sync across them....you have to Dropbox the information which can be a problem when two iPads are filling out a form at the same time. This app would be above and beyond the others if it offered iCloud. I would have paid money just for this feature!

App crashes and is very slow. I used this app for the past year and it has been very frustrating. When entering values it freezes intermittently and then crashes. There are times when I loose my data. Avoid using this app if you value your time. No calculation. Ever since IOS 7. The bugs and issues are even worst.
